If we don’t accept your insurance — or you prefer to skip insurance entirely — we offer self-pay rates and provide superbills on request for out-of-network reimbursement.

Good Faith Estimate.

Under the No Surprises Act, uninsured and self-pay patients have the right to a written estimate of what scheduled care will cost. We issue one before your first visit and update it whenever your treatment plan changes.

Appointment 10+ business days out

Your Good Faith Estimate is provided within 3 business days of the day you schedule.

Appointment 3–9 business days out

Your Good Faith Estimate is provided within 1 business day of the day you schedule.

You ask before scheduling

Request an estimate at any time and we provide it within 3 business days of the request, whether or not you book.

Plan or pricing changes

If your treatment plan changes materially, we issue a revised estimate before the affected visit.

Please read before booking a same-week appointment

Appointments scheduled fewer than 3 business days before the visit fall outside the No Surprises Act timeline, and federal rules do not require a Good Faith Estimate for them. If you book that close in, you may attend your visit without a written estimate in hand.

Please book short-notice appointments with that in mind. If cost certainty matters to you, call 541-224-8110 for a verbal cost range before your visit, or schedule at least three business days out so a written estimate can be issued. You may also request a Good Faith Estimate at any point — including after booking — and we will provide one as promptly as we can.

If your final bill is at least $400 more than your Good Faith Estimate, you may dispute the charge through the federal patient-provider dispute-resolution process. Keep a copy of your estimate. More information is at cms.gov/nosurprises.
