Patient portal

Existing patients can message the office, request refills, view visit notes, complete intake or follow-up forms, pay invoices, and check upcoming appointments in one secure place.

First-time users receive an email invitation once a first visit is scheduled. Trouble signing in? Call (503) 395-4064.

Self-report screeners

Four validated, public-domain questionnaires used in adult psychiatric care. Each takes a few minutes. Print your results and bring them to a first visit, or just take a look to see whether something is worth a conversation.

These are screeners, not diagnoses. A clinical evaluation looks at the bigger picture and at what screening tools cannot see.

Common questions

Answers to what patients ask most.

New patients

How do I become a patient?

Book online from the scheduling page, or call (503) 395-4064. You will complete secure intake forms before the visit.

What does a first visit include?

A comprehensive psychiatric evaluation covering psychiatric and medical history, prior medication trials, sleep, substance use, and current functioning. It is scheduled at extended length rather than in a brief slot.

Do I need a referral?

Most plans do not require one. Some HMOs do, and we will help you obtain it at intake.

Telehealth in Oregon and Washington

Which states do you see patients in?

Oregon and Washington only. You need to be physically located in one of those states during the visit.

What if I travel?

If you are temporarily out of state, message us. We may need to reschedule to stay compliant with licensing rules.

What technology do I need?

A modern browser on any device with a camera and microphone. There is no app to download.

ADHD and controlled-substance prescribing

Do you prescribe stimulants by telehealth?

In most cases yes, under the federal telemedicine rules currently in effect. A full diagnostic evaluation comes first, including validated rating scales and a check of the state prescription monitoring program.

Do you require an in-person visit for stimulants?

Not as a blanket rule. We follow federal and state requirements and, in some cases, request an in-person visit based on clinical judgment.

Insurance and billing

Which plans do you accept?

Moda, Regence BlueCross BlueShield, PacificSource, Providence, First Choice Health Network, MultiPlan, and select Kaiser Permanente plans (PPO, Added Choice, and Dual Choice).

What if you do not take my insurance?

Call the office for current self-pay rates. We provide superbills for out-of-network reimbursement.

Do you accept Medicaid or the Oregon Health Plan?

No. We cannot bill Medicaid, the Oregon Health Plan, Washington Apple Health, or any Medicaid managed-care plan. Self-pay is available if you would like to be seen without insurance.

Will I know my cost before the first visit?

Yes. We verify your benefits before you are seen. If you are uninsured or self-pay, you are entitled to a Good Faith Estimate under the No Surprises Act. See the timelines and what to expect.

Medication and refills

How do I request a refill?

Message us through the patient portal, or ask your pharmacy to send an electronic request. We respond within one business day.

How often do I need follow-ups?

Your first follow-up is about a week after the initial visit. While a dose is being adjusted we meet every two weeks, then monthly once you are stable.

How long are follow-up visits?

Follow-ups are scheduled with adequate time for review and discussion. Routine medication-management visits tend toward the shorter end; visits involving medication changes or several topics tend toward the longer end.

Records and privacy

How do I request my records?

Submit a written request through the portal. Records are released within 10 to 15 business days.

Is my information kept private?

Yes. We follow HIPAA and Oregon and Washington state confidentiality laws. The Notice of Privacy Practices below describes how we use and disclose protected health information.
