A HIPAA-compliant web form processed by Charm Health. You can attach records, list current medications, and flag clinical urgency. Submissions route directly to Lavena and the front office.

Privacy: This form is transmitted over TLS and processed through Charm Health, a HIPAA-compliant healthcare platform with a signed Business Associate Agreement. PHI submitted through this form is treated with the same protections as other clinical records.

We’re a good fit for

  • Adults 18+ with adult ADHD requiring evaluation or established management
  • Major depression and persistent depressive disorder
  • Generalized anxiety, panic disorder, social anxiety
  • PTSD and trauma-related psychiatric care
  • Perinatal mood and anxiety disorders
  • Patients seeking unhurried medication management with measurement-based care
  • Stable patients needing a thoughtful prescriber after losing their previous one

Outside our scope

  • Pediatric and adolescent psychiatry
  • Active or unstable bipolar I with psychotic features
  • Primary substance use disorders requiring integrated addiction medicine
  • Eating disorders requiring higher levels of care
  • Inpatient or partial-hospitalization level needs
  • Court-ordered or forensic evaluations
  • Disability paperwork as the primary purpose of care
