What this website does not collect

This website has no contact forms, intake forms, symptom questionnaires, or account registrations of its own. Our pages never ask for your name, email address, phone number, date of birth, insurance details, or a description of your symptoms.

Simply reading these pages does not send us anything about you personally and does not create a patient record. Analytics records that a page was viewed, as described below, but not who viewed it.

One tool on the site does send us information, but only if you choose to use it: an appointment booking widget appears on every page. It asks a short series of click-to-answer scheduling questions, and your answers reach our office only after you press send. It accepts no free-text description of symptoms.

Appointment booking widget

A small chat bubble appears in the corner of every page on this site, offering to help you book an appointment. It is operated by Charm Health, the vendor that runs our electronic health record, and it loads on all pages — not only the booking pages.

The widget does one thing: it books appointments. It works as a short sequence of set questions that you answer by clicking the options presented — there is no open message box for describing symptoms, no way to tell it how you are feeling, and no free-text conversation with our office. Nothing is transmitted while you are clicking through the questions; your answers are sent only when you press send to submit the appointment request.

What reaches us is an appointment request: the scheduling details you selected and the contact information needed to confirm the visit. Charm Health is a HIPAA-compliant healthcare platform operating under a signed Business Associate Agreement, so a request you submit is handled as protected health information under our Notice of Privacy Practices — the same protection that applies when you telephone the office.

Because the widget is loaded from Charm Health’s servers, opening any page on this site sends your IP address and basic request information to Charm Health even if you never click the bubble. The widget may also store its own cookies or browser data to keep track of a conversation in progress.

Two practical notes. First, you never have to use it: everything it does can be done by calling 541-224-8110. Second, it is a scheduling tool and no one is monitoring it in real time, so please do not use it in an emergency. If you are in crisis, call or text 988, or call 911.

Analytics

This website uses Google Analytics 4 on every page. It is the one tool here that does collect information about you automatically, and we want to be plain about it rather than claim the site is analytics-free.

Google Analytics sets cookies in your browser and reports to Google, on our behalf, information including:

• the pages you view and how long you spend on them
• the site or search engine that referred you
• your approximate location, derived from your IP address and generalized to a city or region
• your device type, operating system, and browser
• a randomly generated identifier that lets Google recognize the same browser across visits

We use this to understand which pages are useful, which are confusing, and where visitors come from. We have not enabled Google Signals, advertising personalization, or remarketing, and there is no advertising pixel — no Meta, TikTok, LinkedIn, or Google Ads tracking — anywhere on this site.

Important limitation of any web analytics on a psychiatry site: the page addresses themselves are descriptive. If you read a page about adult ADHD or depression, the address of that page is part of what analytics records. We do not connect that data to your identity, and we do not attempt to; but if you would rather it not be recorded at all, the opt-out options under Your choices work reliably.

Third-party services on these pages

Several pages load resources from other companies. When your browser requests a resource from another company, that company necessarily receives your IP address and basic request information. The services in use are:

Google Fonts

Typefaces load from Google’s font servers on every page. Google receives your IP address as part of that request.

Charm Health — booking widget

Loads on every page. See Appointment booking widget above for what it collects and when.

Charm Health — scheduling calendars

Our appointment-request calendars are embedded from Charm Health on the contact and telehealth booking pages. Anything you enter into those calendars goes to Charm Health under the same Business Associate Agreement — not into this website — and is covered by HIPAA and our Notice of Privacy Practices.

Charm patient portal

Portal links take you to Charm’s own site, which has its own privacy terms and its own login.

Directory and social links

Links to Psychology Today, Mental Health US, Instagram, and YouTube take you to those companies’ sites. Our Psychology Today and Mental Health US verification badges load images from those companies, and clicking a badge notifies the directory that the click happened. We receive no information about what you do after you leave.

We do not control these companies’ privacy practices. Their own policies apply once a request reaches them.

Self-report screening tools

Our screening questionnaires — PHQ-9, GAD-7, ASRS, and the others — run entirely in your browser. Your answers and your score are never transmitted to us, never saved to a server, and never attached to any record. Closing the tab discards them. If you want a copy, use the print button on the results page.

One caveat worth stating: the fact that you visited a screener page is recorded by analytics in the same way as any other page. Your answers are not.

Cookies and browser storage

Cookies on this site are set by Google Analytics and may also be set by the Charm Health booking widget to keep track of a booking in progress. Neither we nor these tools place advertising cookies, cross-site tracking cookies, or any cookie that identifies you by name.

The site itself also uses your browser’s local storage for small display preferences. That data stays on your device and is never sent anywhere. Clearing cookies and site data for this domain removes everything stored by the site and its embedded tools.

How we use website information

Analytics information is used to improve the website: to see which pages people actually read, to find pages that are broken or unhelpful, and to understand which parts of Oregon and Washington our visitors come from so we can write content that serves them. That is the whole purpose.

We do not use website analytics to build profiles of individuals, to target advertising, to make any clinical judgment, or to make any decision about an individual’s care.

Sharing and selling

We do not sell your information. We do not share it with data brokers, advertising networks, or marketing companies.

Website information is shared only with the service providers described above, in the ordinary course of those services operating, and only where the law requires disclosure — for example, a valid court order.

Your choices

• Block analytics. Google publishes a browser add-on that opts you out of Google Analytics on every site, available at tools.google.com/dlpage/gaoptout.
• Use tracking protection. Firefox, Safari, and Brave block analytics cookies by default; most content blockers block Google Analytics outright. The site works normally with analytics blocked.
• Browse privately. A private or incognito window discards cookies when you close it.
• Clear what is stored. Clearing cookies and site data for this domain removes everything this site has stored on your device.
• Ignore or block the booking widget. You are never required to use it, and content blockers that block third-party scripts will stop it loading. The rest of the site works normally without it.
• Skip the website. You can reach us entirely by phone at 541-224-8110 without using this site at all.

State privacy rights

Washington residents. Washington’s My Health My Data Act gives you rights over consumer health data, including the right to know what is collected, to withdraw consent, and to request deletion. We do not sell consumer health data or share it for advertising. An appointment request you submit through the booking widget is held as part of your health record and protected under HIPAA; screener responses never leave your browser at all. To exercise any right under the Act, or to ask what we hold, contact us using the details below.

Oregon residents. Under the Oregon Consumer Privacy Act you may request access to, correction of, or deletion of personal data we hold about you, and may opt out of its sale or use for targeted advertising. We do not sell personal data or use it for targeted advertising.

Health information held as part of your medical record is governed by HIPAA rather than these state laws, and those requests are handled through the process in our Notice of Privacy Practices.

Children

MindHealth Psychiatry treats adults aged 18 and older. This website is not directed to children, and we do not knowingly collect information from anyone under 13.

Changes to this policy

If we add a tool that collects information differently — a contact form, an advertising pixel, a new embedded service — we will update this page and change the effective date above before or when that tool goes live.
