Acceptance of these terms

mindhealthpsychiatry.net is operated by MindHealth Psychiatry LLC, doing business as MindHealth Psychiatry. By using this website you agree to these terms. If you do not agree with them, please do not use the site. You can reach us by telephone at (503) 395-4064 without using this website at all.

These terms govern the website. They do not govern the clinical care we provide to established patients. That care is governed by the consent and financial documents you sign at intake, by our Notice of Privacy Practices, and by the law of the state in which you receive care. Where those documents and these terms differ, those documents control.

Educational content, not medical advice

The articles, condition pages, medication explainers, and other written material on this site are published for general education. They are not medical advice, and they are not a substitute for evaluation by a clinician who knows your history.

Psychiatric care is individual. A medication that suits one person may be wrong or unsafe for another, dosing depends on factors this website cannot know about you, and general descriptions of conditions cannot account for your particular circumstances. Never start, stop, or change a prescribed medication based on something you read here. Never delay seeking care, or disregard advice a clinician has given you, because of information on this site.

If you are an existing patient, questions about your own treatment belong in a visit or a portal message, not in an interpretation of a public web page.

Using this site does not make you a patient

Reading these pages, taking a screener, browsing a location or insurance page, or submitting an appointment request does not create a provider-patient relationship between you and MindHealth Psychiatry or any of our clinicians.

That relationship begins only when we have accepted you as a patient, verified your eligibility, completed the necessary consents, and you have attended a scheduled initial evaluation with one of our clinicians. Until that happens, no clinician here is responsible for your care and no one here is monitoring your condition.

Submitting an appointment request is a request. It is not a confirmed appointment and it is not acceptance of you as a patient. We may be unable to accept a request for reasons including licensure, insurance, clinical scope, or capacity.

Emergencies and crisis care

This website, the booking widget, email, and social media are not monitored around the clock and are not appropriate for emergencies. Messages sent through any of them may not be seen for some time.

If you are in crisis or are having thoughts of harming yourself or someone else, call or text 988 at any hour, call 911, or go to your nearest emergency department. Established patients should follow the after-hours instructions given at intake.

Self-report screening tools

The questionnaires on this site, including the PHQ-9, GAD-7, ASRS, Mini-SPIN, and PCL-5, are validated, publicly available self-report instruments. They are offered as a starting point for a conversation.

A screener score is not a diagnosis. These instruments were designed to flag symptoms that merit clinical attention, not to identify a condition. They produce both false positives and false negatives. A low score does not mean nothing is wrong, and a high score does not establish that you have a particular disorder. Only a clinical evaluation can do that.

Your answers run entirely in your browser and are never transmitted to us. We do not receive your responses or your score, we cannot see that you took a screener, and no record of it is created. Because of that, taking a screener is not a way to tell us anything, and it does not put you in contact with a clinician.

Appointment requests and scheduling

The booking widget and the embedded scheduling calendars on this site are operated by Charm Health, the vendor that runs our electronic health record. Submitting a request through either one sends an appointment request to our office. Our staff review requests during business hours, Monday through Friday, 8am to 4pm Pacific.

Availability shown on a calendar is not a guarantee. Times can be taken by another patient between the moment a page loads and the moment you submit, and a request may need to be rescheduled after we verify insurance, licensure, or clinical fit. Your appointment is confirmed only when you receive confirmation from our office.

Wait times described anywhere on this site are general estimates based on recent experience. They are not promises, and actual availability varies with demand, clinician schedules, and the type of visit requested.

Insurance, cost, and Good Faith Estimates

Insurance information on this site describes the plans we are generally contracted with. It is not a determination of your individual benefits. Network participation changes, plans within the same carrier differ, and a carrier being listed here does not mean your particular plan covers our services or that a given visit will be paid.

You are responsible for confirming your own coverage with your insurer. We verify benefits before your first visit as a courtesy, and that verification is also not a guarantee of payment. Coverage is determined by your insurer under your plan documents.

Uninsured and self-pay patients have a right to a Good Faith Estimate of expected charges under the No Surprises Act. The process and timing are described on our insurance page. Any figure appearing elsewhere on this site is illustrative and is not a Good Faith Estimate.

Who may use this site

This website is intended for adults aged 18 and older. It is not directed to children, and MindHealth Psychiatry does not treat patients under 18.

We are licensed to practice in Oregon and Washington only. To receive care from us, including telehealth, you must be physically located in Oregon or Washington at the time of your appointment. Nothing on this site is an offer to provide care in any state where our clinicians are not licensed.

Accuracy and availability

We make a genuine effort to keep this site accurate and current, and we correct errors when we find them. Even so, clinical guidance evolves, insurance contracts change, medication information is updated, and pages can fall out of date between reviews. Content is provided as it stands at the time you read it.

We do not guarantee that the site will be available without interruption or free of error, and we may change, suspend, or remove any part of it at any time without notice.

Third-party links and services

This site links to and embeds services operated by other companies, including the Charm Health booking widget, scheduling calendars, and patient portal, Psychology Today, Mental Health US, and social media platforms.

We do not control those services and are not responsible for their content, availability, or practices. Once you follow a link or submit information through an embedded tool, that company’s own terms and privacy policy govern. A link is not an endorsement, and the provider directories we link to are not screened or recommended by us individually.

Content ownership

The written content, layout, design, photographs, and the MindHealth Psychiatry name and logo on this site are owned by us or used under license, and are protected by copyright and trademark law.

You are welcome to read, print, and share our pages for your own personal, non-commercial use, and to link to them. Republishing our written content, using it to train commercial models, or using our name or logo in a way that suggests affiliation or endorsement requires our written permission. Insurance carrier names and logos appearing on this site are the property of those carriers and are used to describe network participation only.

Acceptable use

Please do not use this website to transmit anything unlawful or harassing, to attempt to gain unauthorized access to our systems or those of our vendors, to scrape or harvest content at scale, to interfere with the site’s operation, or to submit false information in an appointment request. We may restrict access to anyone who does.

Please also do not send protected health information to us by ordinary email or social media. Those channels are not secure. Use the patient portal, the booking widget, or the telephone.

Privacy

How this website handles visitor information, what analytics records, and how that differs from the medical record are described in our Website Privacy Policy. Protected health information is governed by HIPAA and described in our Notice of Privacy Practices.

Disclaimer of warranties

This website and its content are provided on an as-is and as-available basis. To the fullest extent permitted by law, we disclaim all warranties, express or implied, including any implied warranty of merchantability, fitness for a particular purpose, accuracy, or non-infringement.

This disclaimer applies to the website. It does not limit the professional duty we owe to our established patients in the course of their clinical care.

Limitation of liability

To the fullest extent permitted by law, MindHealth Psychiatry and its clinicians and staff are not liable for any indirect, incidental, or consequential damages arising from your use of this website or your reliance on its general educational content.

Nothing in these terms limits any liability that cannot be limited under applicable law, and nothing here waives, limits, or alters any right you have as a patient, including any right relating to the professional care we provide.

Governing law

These terms are governed by the laws of the State of Oregon, without regard to its conflict-of-laws rules. Any dispute relating to this website will be brought in the state or federal courts located in Marion County, Oregon. If any provision of these terms is found unenforceable, the rest remain in effect.

Changes to these terms

We may update these terms as the site changes. When we do, we will revise the effective date at the top of this page. Continuing to use the site after a change means you accept the revised terms.
